CCSD Health Screenings
The Pennsylvania School Health Code mandates health screenings in every grade. Below is a description of what services will be performed for each grade level.
If you would prefer your student to have these completed by their private doctor, at your expense, please notify the school nurse of your student’s building to obtain the necessary forms.
It is highly encouraged for your student to see their private doctor and dentist. If you need assistance in finding one of these please notify the school nurse of your student’s building.
Both in school physical and dental exams are provided by the district if a student is unable to be assessed by their private provider.

Cameron County School District Health Forms

Student Health History
This form is used upon initial enrollment to the Cameron County School District.

Annual Student Health Update
Every year this form should be updated and returned to the school nurse to assure we have current information on your student's health status.

Medication Administration Consent
In accordance with school policy, medication(s) should be given at home before and/or after school. However, when this is not possible, prior to receiving the medication at school, each student must provide the school nurse with a Medication Administration Consent form signed by the student’s parent/guardian and a Medication Order from a licensed prescriber. All medications must be in an original prescription bottle/container from a pharmacy.

Private Physical Exam
Pennsylvania School Health Code 23.2 - Student's are encouraged to see their family doctor for continuity of care and for a more complete physical. Any physical done by the student's family doctor within 1 year of the start of the school year will meet this requirement. Have the student's family doctor complete and return this form to the health office.

Private Dental Exam
Pennsylvania School Health Code 23.3 - Students are encouraged to see their family dentist for continuity of care. Any dental exam completed by the student's family dentist within 1 year of the start of the school year will meet this requirement. Have the student's family dentist complete and return this form to the health office.

PIAA Sports form
A new packet containing sections 1-6 must be completed each year starting May 1st for all students planning to participate in athletics. This must be complete and on record in the CCHS health office prior to your student participating.

Medical Plan of Care for School Food Service
Individual Healthcare Plan to be completed by physician and signed by parent of student whose disability requires modification in provided school meals.

Type 1 Diabetes Parent Fact Sheet
Pennsylvania School Code 24 P.S. § 1414.12. – This form is to provide parents and guardians of incoming elementary school students and students entering grade six information regarding Type 1 diabetes in children. Type 1 diabetes in children is an autoimmune disease that can be fatal if untreated, and this information is intended to help raise awareness regarding this disease.
